Verifying your insurance coverage

From time to time, we might ask you to provide information about your current insurance coverage so we can verify that:

• MIDFLORIDA is listed as the lienholder (or loss payee).
• Your coverage includes comprehensive and collision with a deductible that does not exceed $1,000.

You can provide us with your policy information by choosing one of the following options:

• Have your insurance agent call our Insurance Service Center at 800-653-8812.
• Visit our verification website at www.myinsuranceinfo.com
• Mail a copy of your policy to our Insurance Department at PO Box 8008; Lakeland, FL 33802.
