- Guardianship documents

Medallion Signature Guarantee
Medallion Signature Guarantee services are offered to current members as follows:
- Up to $500,000 (all branches) – We recommend scheduling an appointment online to see a Member Service Representative and minimize any potential wait time. Please note in the instructions box “Medallion Signature Guarantee”.
- From $500,000.01 to $2,000,000 (select branches) – Please contact our Help Desk at 863-688-3733 for available locations.
Please note: A recent statement for the security document is required that includes the following information:
- Name of Security Owner(s)
- Title of Security
- Company Issuing Document
- Company Issue Date
- CUSIP Number
- Number of Shared Represented by the Certificate
- POA, if applicable
