From time to time, we might ask you to provide information about your current insurance coverage so we can verify that MIDFLORIDA is listed as the "Additional Insured" and that your coverage includes comprehensive and collision with a deductible that does not exceed $1,000.

Have your insurance agent call our Insurance Service Center at (800) 653-8812.
Mail a copy of your policy to our Insurance Department at PO box 8008, Lakeland, FL 33802.
